IMPORTANT INFORMATION FOR DR.SUE'S PATIENTS

Dr. Sue regrets to inform her patients that, due to her physical health, she will not be returning to clinical practice in the near future.  Unfortunately, Dr. Sue's patients are advised to establish themselves with other family physicians. 

Patients can telephone The Department of Health at (709) 729-4984 to obtain a complete list of family physicians accepting new patients.  Similarly, a list of family physicians accepting new patients can be obtained at:

Dr. Sue will make every effort to respond to the large volume of emails and telephone messages she has received requesting medication refills and medical guidance via correspondence.  However, after September 30, 2010 she will not be available for these services.
 
Unfortunately, due to an already saturated practice, “Dr. Steve” is unable to assume the care of Dr. Sue's patients.  Similarly, Dr. Steve is unable to provide medical advice and prescription refills for Dr. Sue's patients.
 
Patients of Dr. Sue who require immediate prescription refills and/or medical care may consider using 'walk-in' clinics until they have established themselves with their new physicians.

To ensure the continuity of medical care for Dr. Sue’s patients, we will work diligently to provide the efficient transfer of appropriate medical information, charts and files.  There will be no charge for this service.

A signed consent form is essential for the transfer of medical files/information.  To facilitate the tranfer of your medical records to your new physician please forward us a signed chart transfer request by fax:  (709) 782-4447.
 
Dr. Sue deeply apologizes for any inconvenience to her valued patients.  She would also like to express what an incredible honour it has been to care for her patients over the years.  Similarly, she would like to express her heartfelt thanks for her patients' understanding and patience.
